Product Update Form Description

The following modifications are made: 1) New fields 'Gender Identity', 'Gender Identity (Please Describe)', 'Sexual Orientation' and 'Sexual Orientation (Please Describe)' have been added to the 'Update Client Data' form and the 'Demographics' section of the 'Admission', 'Admission(Outpatient)', 'Discharge', 'Discharge (Outpatient)', 'Pre Admit' and 'Pre Admit Discharge' forms. Please note that form designer changes are also included for the forms mentioned. 2) Field 'Are you heterosexual, lesbian, gay, bisexual, transgender or do you question your sexual orientation?' is removed from the form(s)/section(s) listed in item 1 and is defined as an extended dictionary of field 'Sexual Orientation'. 3) The 'Preferred Name' field that a part of the form(s)/section(s) listed in item 1 is modified to support apostrophes and hyphens. 4) The 'Sex' values Transgender (F to M)' and Transgender (M to F)' are made inactive. 5) The 'ClientAdmission' and 'ClientDemographics' web services are modified to support these changes. 6) Fields 'Veteran', 'Military Branch Of Service', 'Military Service From', 'Military Service To' and 'Military Related Disability' are removed from the 'Other Client Data' section of the 'Admission' and 'Admission (Outpatient)' forms and added to the form(s)/section(s) listed in item 1. 7) The check for female in the 'Women's Health History' and 'Cost Reporting Data Maintenance' forms is modified to only verify that the client's sex is 'Female'. 8) The following tables are modified to support these changes: 'SYSTEM.patient_current_demographics', 'SYSTEM.patient_demographic_history', 'SYSTEM.view_episode_summary_admit', 'SYSTEM.view_episode_summary_current', 'SYSTEM.view_episode_summary_discharge' 9) Please note that this update includes a client data conversion process to a) Populate 'Gender Identity' based on 'Sex'. b) Convert 'Sex' code 'Transgender (M to F)' to 'Male' and 'Transgender (F to M)' to 'Female'. c) Populate 'Sexual Orientation' based on 'Are you hetero...'. d) Store all veteran related information by client instead of by episode. Avatar Cal-PM Sexual Orientation/Gender Identification/Veteran Field Updates
Updates are made for Sexual Orientation/Gender Identification/Veteran fields across several Avatar Cal-PM forms.
The following modifications are made:

* Fields 'Gender Identity', 'Gender Identity (Please Describe)', 'Sexual Orientation' and 'Sexual Orientation (Please Describe)' have been added to the 'Update Client Data' form and the 'Demographics' section of the 'Admission', 'Admission(Outpatient)', 'Discharge', 'Discharge (Outpatient)', 'Pre Admit' and 'Pre Admit Discharge' forms. Please note that form designer changes are also included for the forms mentioned.

* Field 'Are you heterosexual, lesbian, gay, bisexual, transgender or do you question your sexual orientation?' is removed from the form(s)/section(s) listed in first item above and is defined as an extended dictionary of field 'Sexual Orientation'.

* The 'Preferred Name' field in the form(s)/section(s) listed in first item above is modified to support apostrophes and hyphens.

* The 'Sex' values Transgender (F to M)' and Transgender (M to F)' are made inactive.

* The 'ClientAdmission' and 'ClientDemographics' web services are modified to support the Sexual Orientation/Gender Identification changes above.

* Fields 'Veteran', 'Military Branch Of Service', 'Military Service From', 'Military Service To' and 'Military Related Disability' are removed from the 'Other Client Data' section of the 'Admission' and 'Admission (Outpatient)' forms and added to the 'Update Client Data' form and the 'Demographics' section of the 'Admission', 'Admission(Outpatient)', 'Discharge', 'Discharge (Outpatient)', 'Pre Admit' and 'Pre Admit Discharge' forms.

* The check for female in the 'Women's Health History' and 'Cost Reporting Data Maintenance' forms is modified to only verify that the client's sex is 'Female'.

* Logic for 'Cal-OMS Submission' Admission file 'Lesbian/Gay/Bisexual/Transgender (LGBT)' field (CID-20) is updated for new Client Demographic 'Sexual Orientation' field/dictionary values.

* The following Avatar Cal-PM SQL tables are modified to support these changes: 'SYSTEM.patient_current_demographics', 'SYSTEM.patient_demographic_history', 'SYSTEM.view_episode_summary_admit', 'SYSTEM.view_episode_summary_current', 'SYSTEM.view_episode_summary_discharge'

Avatar Cal-PM Sexual Orientation/Gender Identification/Veteran Information Conversion
A client data conversion process for the Avatar Cal-PM Gender Identity, Sexual Orientation and Veteran fields is included in Avatar Cal-PM 2022 Update 80.
This update includes a client data conversion process to do the following:
* Populate 'Gender Identity' based on 'Sex'
* Convert 'Sex' code 'Transgender (M to F)' to 'Male' and 'Transgender (F to M)' to 'Female'
* Populate 'Sexual Orientation' based on 'Are you heterosexual, lesbian, gay, bisexual, transgender or do you question your sexual orientation?'
* Store all veteran related information by client instead of by episode
Value Added: Converts existing client data for updated Sexual Orientation/Gender Identification/Veteran fields.

Scenario 1: Avatar Cal-PM 'Dictionary Update' - Verification of Demographic Gender Identity/Sexual Orientation Fields
Steps
  1. Open Avatar Cal-PM 'Dictionary Update' form.
  2. Navigate to 'Print Dictionary' section of form.
  3. Select 'Client' File.
  4. Select 'All Data Elements' or select 'Individual Data Element' and select one of the updated Data Elements/dictionaries noted below.
  5. Click 'Print Dictionary' button to display dictionary fields/values for selected Data Element.
  6. Confirm Avatar Cal-PM 'Client' File Data Elements/Dictionary Codes are updated/present as follows:

  1. Open Avatar Cal-PM 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form.
  2. In Client Search form, enter values for client search criteria and click 'Search' button.
  3. Click 'New Client' button for Admission form entry/new client creation (or select Client Search result row for existing client Admission entry/update).
  4. Enter/select values for 'Client Name', 'Sex', 'Preadmit/Admission Date', 'Preadmit/Admission Time', 'Program', 'Type Of Admission', 'Admitting Practitioner' and any other required/desired fields in main form section.
  5. Navigate to 'Demographics' section of form.
  6. Ensure that the following fields are present in 'Demographics' section of 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form:
  7. 'Gender Identity'
  8. 'Gender Identity (Please Describe)'
  9. 'Sexual Orientation'
  10. 'Sexual Orientation (Please Describe)'
  11. Select value in 'Gender Identity' field.

  13. Ensure that the ‘Sexual Orientation’ field is conditionally required/not required based on selected Program value (according to ‘Require Sexual Orientation Response During Admission?’ field Avatar Cal-PM ‘Program Maintenance’ form).
  14. Select value in 'Sexual Orientation' field.
  15. Ensure that the 'Sexual Orientation (Please Describe)' field is enabled and required if value 'Something else, please describe' is selected in 'Sexual Orientation' field; enter value in 'Sexual Orientation (Please Describe)' field if enabled/required.
  16. 'Ensure that the 'Are you heterosexual, lesbian, gay, bisexual, transgender or do you question your sexual orientation?' field is not present/is removed from 'Demographics' section of 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form.
  17. Enter value in 'Preferred Name' field.
  18. Ensure that if invalid spacing/characters are included in 'Preferred Name' value, the entry is disallowed and user is presented with an error dialog noting 'Invalid Format: Limited to alphabetic characters with an optional intervening space, hyphen or apostrophe.'
  19. Enter/select values for any other fields in form as required/desired.
  20. Click 'Submit' button to file 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form.
  21. Re-open Avatar Cal-PM 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form for same client/episode as filed above.
  22. Navigate to 'Demographics' section of form.
  23. Ensure that previously filed values are present in 'Gender Identity', 'Gender Identity (Please Describe)', 'Sexual Orientation', 'Sexual Orientation (Please Describe)' and 'Preferred Name' fields.
  24. Note - Client Demographic field values filed in 'Admission', 'Admission (Outpatient)' or 'Pre Admit' forms may also be confirmed via the Avatar Cal-PM 'Update Client Data' form.
  25. Open Crystal Reports or other SQL reporting tool.
  26. In Avatar Cal-PM SQL tables 'SYSTEM.patient_current_demographics' and 'SYSTEM.patient_demographic_history', ensure that values are present for all information filed via Avatar Cal-PM 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form including the following fields:

  • Crystal Reports or other SQL reporting tool
Steps
  1. Open Avatar Cal-PM 'Update Client Data' form.
  2. Select existing client for Demographic information update.
  3. Ensure that the following fields are present in 'Demographics' section of 'Update Client Data' form (main form section):
  4. 'Gender Identity'
  5. 'Gender Identity (Please Describe)'
  6. 'Sexual Orientation'
  7. 'Sexual Orientation (Please Describe)'
  8. Ensure that fields noted above are populated with existing Client Demographic values/information in form display.
  9. Select value in 'Gender Identity' field.
  10. Ensure that the 'Gender Identity (Please Describe)' field is enabled and required if value 'Additional gender category or other, please specify' is selected in 'Gender Identity' field; enter value in 'Gender Identity (Please Describe)' field if enabled/required.
  11. Select value in 'Sexual Orientation' field.

  1. Open Avatar Cal-PM 'Discharge', 'Discharge (Outpatient)' or 'Pre Admit Discharge' form.
  2. Select existing client/episode for Discharge entry.
  3. Enter/select values for 'Date Of Discharge', 'Discharge Time', 'Type Of Discharge', 'Discharge Practitioner' and any other required/desired fields in main form section.
  4. Navigate to 'Demographics' section of form.
  5. Ensure that the following fields are present in 'Demographics' section of 'Discharge', 'Discharge (Outpatient)' or 'Pre Admit Discharge' form:
  6. 'Gender Identity'

  14. Select value in 'Military Related Disability' field (Yes/No).
  15. Select value in 'Military Branch Of Service' field.
  16. Ensure that the 'Military Service From' and 'Military Service To' fields are enabled if value is selected for 'Military Branch Of Service' field; enter value(s) in 'Military Service From' and 'Military Service To' fields if enabled/desired.
  17. Enter/select values for any other fields in form as required/desired.
  18. Click 'Submit' button to file 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form.
  19. Re-open Avatar Cal-PM 'Admission', 'Admission (Outpatient)' or 'Pre Admit' form for same client/episode as filed above.
  20. Navigate to 'Demographics' section of form.
