myAvatar login page

Note: Any unattended workstation that is logged into myAvatar is a potential security risk. If you leave your workstation, be sure to log out.

There are four fields on the myAvatar login page:

References the application (PM, Cal-PM, or MSO). The value should be automatically filled. If not, select from the drop-down list.

 

 
System Code This is the agency name/acronym you want to access. The "live" system code automatically populates and can be changed (if multiple system codes are available).
User ID This is provided to you by your agency but is often first initial last name. It is created by a system administrator in the User Definition form.
Password A temporary password is provided to you at first login. You must reset your password to ensure HIPAA confidentiality. If you forget your password, a new temporary password can be issued by your system administrator.
