About the Diagnosis Form - PM

This form allows you to record a client’s diagnosis record. From this form you can record an admission, discharge, onset, or update for a diagnosis. (Diagnosis data is filed by date and uses DSM-5 and ICD-10 codes).

This form has logic built in that will update fields and require different information as you make selection and fill in data. Be sure to fill out as much information as possible and ensure all data is entered correctly before saving. Once the form is submitted you can no longer delete a diagnosis but you can void it.

The client's primary diagnosis (The reason they are being treated) should be listed as the first diagnosis to ensure billing is appropriately configured. If a diagnosis is entered out of order, it can cause claims to be rejected. 
Note: The standard Diagnosis form supports an unlimited number of diagnosis records. Modeled Diagnosis forms, however, support a maximum of 15 records.
