ASAM Integration - Finalize ASAM Assessment
The Finalize ASAM Assessment form is a client-based and episodic form accessed in myAvatar NX or ProviderConnect NX to launch the ASAM and complete either a Comprehensive or a Triage Assessment. End users are able to input their own Comments, their Recommended Level of Care and the Actual Level of Care assigned to an individual. This form can also be used to launch to summary or narrative reports generated by ASAM.
This version of the ASAM Assessment form can be finalized and disallow future edits to a specific Comprehensive or Triage Assessment record. Staff such as a supervisor or licensed staff member may use this form to review ASAM documentation completed by another clinician or non-licensed staff member, a user that is responsible for documenting the Actual Level of Care, or a user that is allowed to finalize their own documentation.
The Finalize ASAM Assessment form supports the same functionality as the ASAM Assessment form, with the following differences:

Recommended Level of Care |
This optional field is used to collect a clinician’s choice for the 'Recommended Level of Care' for the selected ASAM Assessment record. After finalizing a record, the Recommended Level of Care cannot be changed. |

Actual Level of Care |
Use this optional field to enter an Actual Level of Care for the selected ASAM Assessment record, should it differ from the value entered in Recommended Level of Care. Configuration:

Level of Care Override |
This field is enabled and required in cases where the 'Recommended Level of Care' and 'Actual Level of Care' values are different (if entered) for the selected ASAM Assessment record. Configuration:

Program |
Use this optional field to enter a suggested Program for completed ASAM Assessment records. This field is used for reporting purposes only. Note: The Program values available for selection are limited to those where Usage Type for the program is 'Service' or 'Both’ (if the myAvatar PM/Cal-PM registry setting 'Enable Admission vs Service Program Functionality' is enabled). After finalizing a record, this field cannot be changed. |

Draft/Final |
Specify whether an ASAM Assessment record is in 'Draft' or 'Final' state. Records in 'Final' state may not be edited with the ASAM Assessment form. Note: Document Routing is not supported on the Finalize ASAM Assessment and ASAM Assessment forms. |

Select this button to save the Recommended Level of Care, Actual Level of Care, and Level of Care Override values for the selected ASAM Assessment record. |
ProviderConnect NX
ProviderConnect NX uses the same configuration and forms as myAvatar NX. No additional configuration is needed.
There are no differences in workflow, functionality, or instruction for the ASAM Assessment and Finalize ASAM Assessment forms other than the differences between the forms as mentioned in the field descriptions.
ProviderConnect
In addition to the myAvatar and CareFabric configuration, ProviderConnect needs to be updated to configure the applicable User Access Group(s) to have access to the ProviderConnect versions of the ASAM Assessment and Finalize ASAM Assessment forms.
The workflow, functionality, and instruction for both the ASAM Assessment and Finalize ASAM Assessment forms in myAvatar apply to the forms in ProviderConnect.
