Disclosure Section - PM
Each disclosure management item selected in the Disclosure Information field (Disclosure section) will be sent to the default printer when the form is submitted. The Disclosure # of Pages field (Disclosure section), is increased by 1.

Disclosure management chart items and document imaging items must be requested and authorized (if applicable) to be available for print in the Disclosure section.
- In the Disclosure Date field, enter the date.
Note: When a date is entered in this field and this option is filed - the disclosure can be viewed but cannot be edited. - In the Disclosure Time field, enter the time.
- In the Status field, select the disclosure status.
- In the Disclosure Processed By field, enter the clinician's name or ID, and select.
- In the Chart Disclosure Information field, select the item. This field is populated with both requested items that do not require authorization (Request section) and items that have been authorized (Authorization section).
- In the Disclosure Images field, select the images.
- In the Disclosure Method field, select the method of disclosure.
Note: If Other is selected, enter clarifying information in the Disclosure Method–Specify Other field. - The Disclosure # of Items field displays the number of times this form was filed for this client.
- In the Cost field, enter the cost for the requested disclosure. (This is only used for reporting purposes.)
- In the Payment Received field, select the status of the payment.
- Click Process to file the disclosure. You will be presented with an opportunity to preview the disclosure packet.
- In the Meaningful Use Clinical Info Exchange field, select the appropriate form of information exchange.
- In the Disclosure Comments field, enter any associated comments.
- Click Submit to finalize your submissions.
