Disclosure Management Use - PM

Request section

Document an external request for disclosure of client information that comes into an agency.

  • Request Date - This date becomes disabled after filing this form. 
  • Process Type - Select the row of data and can be used for reporting purposes.
  • Process Type is setup in the Dictionary Update form (Client file - data element 40001).
  • Requested Chart Items - This field contains a list of Crystal Reports imported using the Import Reports form. These chart items can be printed when the disclosure process is completed. 
  • Requested Document Images - This field contains all images associated with the client.
    Printing of document images is currently not supported through the Disclosure Management form.
  • Request Information – Specify Other - Enter a description of any item requested, other than chart items or document images.
  • Request Information Date Range - Specifies the beginning date and ending date in the information request. If an item requested does not require an authorization, the date range will be passed through to the report run when printing the Chart Item. 
  • Purpose of Information - This is a user-defined dictionary to record the purpose of the information request. 
  • Purpose Of Information is setup in the Dictionary Update form (Client file - data element 40007).
  • Agency Information - Record the agency information making the request. 

Authorization section

Document the chart items the client has authorized to be released.

Authorization is done per request.

This is necessary when chart items are imported with the Does Disclosure Management Chart Item Require Authorization field in Import Reports is set to ‘Yes’.

  • Authorization Origin- Identifies where the authorization to release information originated from.
    Authorization Origin is setup in the Dictionary Update form (Client file - data element 41000).
  • Authorization Episode(s)-Select the client episode. 
  • Authorization Date Range- Specifies the beginning date and ending date in the information authorization request. If an item requested does not require an authorization then the date range provided on the Request section will be passed through to the report. 
  • Chart Items Authorized for Disclosure- This field displays all chart items requested and their authorization status. This field updates when Refresh Chart Items is clicked. This field should be updated after the authorization status of chart items is updated. 
  • Update Chart Items Authorized for Disclosure- Click to launch the worksheet to authorize chart items.

 

  • Click ‘Yes’ and click Save.
  • Click Refresh Chart Items to update the Chart Items Authorized for Disclosure display field.
  • Document Images Authorized for Disclosure- This field displays the documents or images requested. 
  • Update Document Images Authorized for Disclosure- Click to launch the worksheet to authorize the Document Images.
    Click ‘Yes’ and click Save to exit the worksheet.
    ​Click Refresh Document Images to update the Document Image Authorized for Disclosure display field.

 

  • Specify Other-Authorized for Disclosure- Selecting Yes in this field enables the text field to provide additional information. 
  • Reason for Denial- Enter a a reason for denied authorizations, if appropriate.

Disclosure section

Document the chart items the agency has disclosed. Once a disclosure date has been entered and the Disclosure Management form is submitted, no further edits to that record will be allowed.

Items that were authorized in the Authorized section can be selected for disclosure. If there are any items that have not been authorized, access to this section will be prevented and a message will display to complete the authorization determination in the Authorization section.

Document images can be viewed or printed from in the Clinical Document Viewer.

  • Disclosure Date- Submitting the Disclosure Management form with a Disclosure Date will finalize the record and prevent future edits. These records can be viewed and printed. 
  • Disclosure Information- This field contain all items that were authorized, or did not require authorization.
  • Select the items to disclose. 
  • Disclosure Method- Indicate how the disclosed information will be delivered. 
  • Disclosure Method is setup in the Dictionary Update form (Client file - data element 42011).
  • Disclosure # of Pages- This field will populate after the form has been submitted with the disclosure date. Once the disclosure has been processed, the Request and Authorization sections will be disabled, and data can be viewed or printed.
  • This field will track the number of times the disclosure was printed.
  • Click Submit to complete the full disclosure.
  • A warning message will display warning that once a Disclosure Date is entered, no further edits will be allowed. Users can view and print the report when entering this record. Click ‘Ok’ to proceed.
  • Any authorized chart items or those that do not require an authorization will be printed.

Disclosure Completed/Pending

Users can access any completed or pending disclosures in the Disclosures Completed/Pending form.

  • Select the appropriate parameters and click Process.
  • The reports will be generated.
