Admission - Other Client Data Section - PM
Record additional client admission information as part of Admission forms.
- In the Current Resident Code field, select the client residence.
- In the Homeless Indicator field, select client homeless status.
- In the Client’s Living Arrangements field, select the client's living situation.
- In the Number Living In Household field, enter the number.
- For the 'Handicap' fields, select client handicaps as appropriate.
- In the 'Current Medications' fields, select client medications as appropriate.
- In the Prescreened field, select Yes if the client was screened before being admitted.
- In the Prescreened By Whom field, enter the person who prescreened the client.
- For the School Record Requested field, select Yes if the record was requested.
- In the Immunization Records Requested field, select Yes if the record was requested.
- In the Informed Of Smoking Policy field, select Yes if the client was informed.
- In the Smoker field, select the client's smoking status.
- In the Veteran field, select Yes if the client is a veteran.
- If applicable, in the Military Related Disability field, select Yes if the client has a disability related to the military.
- If applicable, In the Military Branch Of Service field, select the client's military status.
- If applicable, in the Military Service From and Military Service To fields, enter the dates as appropriate.
- For the Criminal Justice Involvement field, select the client's involvement.
- In the Admission Referral Type field, select the type of referral.
- Click Submit.
