Admission - Other Client Data Section - PM

Record additional client admission information as part of Admission forms.


  1. In the Current Resident Code field, select the client residence.
  2. In the Homeless Indicator field, select client homeless status.
  3. In the Client’s Living Arrangements field, select the client's living situation.
  4. In the Number Living In Household field, enter the number.
  5. For the 'Handicap' fields, select client handicaps as appropriate.
  6. In the 'Current Medications' fields, select client medications as appropriate.
  7. In the Prescreened field, select Yes if the client was screened before being admitted.
  8. In the Prescreened By Whom field, enter the person who prescreened the client.
  9. For the School Record Requested field, select Yes if the record was requested.
  10. In the Immunization Records Requested field, select Yes if the record was requested.
  11. In the Informed Of Smoking Policy field, select Yes if the client was informed.
  12. In the Smoker field, select the client's smoking status.
  13. In the Veteran field, select Yes if the client is a veteran.
  14. If applicable, in the Military Related Disability field, select Yes if the client has a disability related to the military.
  15. If applicable, In the Military Branch Of Service field, select the client's military status.
  16. If applicable, in the Military Service From and Military Service To fields, enter the dates as appropriate.
  17. For the Criminal Justice Involvement field, select the client's involvement.
  18. In the Admission Referral Type field, select the type of referral.
  19. Click Submit.
