Patient Notification Menu - Parent/Guardian Information - Cal-PM

Manage parent or guardian client information.

  • This form is for information-only and can be modified using the modeling functionality of RADplus Utilities.

  1. Go to: Avatar PM > Client Management > Client Information > Patient Notification Menu > Parent/Guardian Information
  2. In the Select Client screen, enter the client ID in the Client Name/ID field, click the Search button, and select the corresponding entry.
    • If the client has multiple episodes, the Episode Selection pre-display shows. Select the client episode, click OK.
    • If the client has previous parent/guardian information records, the pre-display dialog displays. Select the client record, click Edit.
  3. In the Parent/Guardian Contact Name field, enter the name in the format LAST,FIRST.
  4. In the Parent/Guardian Relationship field, select the relationship to the client.
  5. In the Parent/Guardian Marital Status field, select the marital status.
  6. In the Parent/Guardian Date Of Birth field, enter the date of birth.
  7. In the Parent/Guardian Social Security # field, enter the social security number.
  8. Access to the Social Security Number field is defined in the User Definition, and User Role Definition forms (Social Security Number Access field). This is defined by a system administrator. The user may be able to:
    • Enter new data for a client, edit and view data.
    • Enter new data for a client, and view data.
    • Enter new data for a client.
  9. In the Parent/Guardian Living With Client field, select Yes if the parent or guardian is living with the client.
  10. In the Parent/Guardian demographic fields, enter the parent or guardian contact information.
  11. In the Parent/Guardian Employer fields, enter the contact information of the parent or guardian’s employer.
  12. Click Submit.
