OSHPD Other Information - Cal-PM

File Office of Statewide Health Planning and Development, OSHPD Medical Information Reporting for California (MIRCal) inpatient data.

  • This form is for inpatient episodes only.
  • This information is included in the client record for an OSHPD Submission.
  • If referral information was previously filed for the client, that information populates and can be edited.
  • If a treatment program is associated with OSHPD, selecting Yes in the OSHPD Program field (Program Maintenance form) allows data to be entered for clients who have been admitted to the OSHPD associated treatment program.

  1. Go to: Avatar PM > Client Management > Client Information
  2. In the Select Client screen, enter the client name or ID in the Client Name/ID field, click the Search button and select the corresponding entry.
    • The Episode Selection pre-display shows if the client has multiple episodes.
    • Select an episode, click Ok.
  3. In the Principal Diagnosis Code field, select the diagnosis code. This list contains diagnoses filed in the OSHPD Diagnosis Code(s) section.
  4. In the Principal Diagnosis Code Present at Admission field, select the principal diagnosis code admission status.
  5. In the Principal Procedure Code field, select the procedure code. This list contains procedure codes filed in the associated OSHPD Procedure Code(s) section.
  6. In the Principal Procedure Code Date field, enter the date.
  7. When finished, click Submit.
