Change Medical Record Number - Cal-PM

Change the client ID number.

  • A specific number can be chosen or a number can be generated.
  • This form is not available for applications with MPI enabled. Facilities with MPI enabled should use the Change MPI MR # form.

  1. Go to: Avatar PM > System Maintenance > Client Maintenance > Change MR#
  2. In the Client ID field, enter the client name, click the Search button, and select the corresponding entry.
  3. The New Client ID field displays the new client ID number that will be assigned when this form is submitted.
  4. Click Assign MR# to display the Auto-Assign dialog to assign a generated client ID number.
  5. Click Submit.
