Treatment - Vaccines - STD module
In the STD module, review and add vaccine history records, including information for the SDIR (San Diego Regional Immunization Registry).
On the main menu navigate to Treatment > Vaccines.
Previous vaccinations - Days since last dose
Review the patient's history of previous vaccines to determine how to proceed with vaccination treatment.
SDIR
- For the Check SDIR item, select Yes if you checked the SDIR for the patient's information.
- For the SDIR information incorporated item, select Yes if you placed information from the SDIR into the patient's record.
Add a vaccine treatment
- Under the Vaccines grid, click Add Entry.
- In the Type box, click the arrow and select a vaccine. For all vaccine types except Influenza, the Vaccine box shows the vaccine name. If Other is selected, enter details in the box provided.
- In the Dose # box, click the arrow and select the dose number.
- In the Lot # box, enter the number.
- For the Expiration Date and VIS Form Date, enter the date. Optionally click the arrows or the calendar icon
to select a date.
- Under Consent
- For the Consent given item, select Yes if the patient granted consent.
- For the Consent uploaded to EHR item, select Yes if you uploaded the consent to the patient's record.
- In the Consent Witness box, click the arrow and select the person who witnessed the patient's consent.
- For the Consent Date, enter the date. Optionally click the arrows or the calendar icon
to select a date.
- Under Administration
- For the Vaccine entered into SDIR item, select Yes if you entered the vaccine information into the SDIR.
- For the Site, click the arrow and select a choice. If Other is selected, enter the site in the box provided.
- In the Vaccinator box, click the arrow and select the person who administered the vaccine.
- For the Date Given, enter the date. Optionally click the arrows or the calendar icon
to select a date.
- Click Save Entry.
- In the Client should return for next vaccine in box, enter the number of days, weeks, or months that indicate when the patient should return. In the next box, click the arrow and select the period of time (days, weeks, or months).
- On the toolbar above the menu, click the Save icon
