Risk Assessment - Personal - STD module
In the STD module, the Personal Risk Assessment form is where you document the patient's sexual encounters to determine possible risks.
On the main menu navigate to Risk Assessment > Personal.
Sexual partners
- For the Ever Had Sex With item, select the check box for each choice that applies to the patient.
- On the toolbar above the menu, click the Save icon

Partner totals
- In the Partner Totals panel, for each type of sexual partner, enter the number of partners for the previous three-month, six-month, and 12-month periods.
- On the toolbar above the menu, click the Save icon
.
Sexual risk assessment
Document the patient's condom use and last sexual encounter.
Condom use by sexual act type
- For each type of sexual act, select the choice that best describes the patient's condom use. If none of the choices apply, select N/A.
- Vaginal Sex
- Anal Insertive
- Anal Receptive
- Received Oral Sex
- Performed Oral Sex
- For the Any receptive anal sex in past 6 months? item, select Yes if applicable to the patient.
- For the Douching past 30 days item, select Yes if applicable to the patient.
- On the toolbar above the menu, click the Save icon

Last sexual encounter
- In the Days Since Encounter, Weeks Since Encounter, and Months Since Encounter boxes, enter the number of days.
- For the Last Date Known, enter the date of the patient's last known sexual encounter. Optionally click the previous and next arrows or the calendar icon
to select a date. To clear the date, click the X.
- On the toolbar above the menu, click the Save icon
