Risk Assessment - Partner - STD module
In the STD module, the Partner Risk Assessment form is where you document the patient's HIV risk based on their sexual partners.
Menu Path
On the main menu navigate to Risk Assessment > Partner.
- For the Patient HIV Testing History item, select the choice that best applies to the patient.
- In the Patient's Last HIV Test Date box, enter the date. Optionally click the previous and next arrows or click the calendar icon
to select a date. To clear the date, click the X.
- Under Sex with HIV + Partner, for the Sex with an HIV + individual item, select Yes if applicable to the patient. If Yes, provide information for the following items.
- Had sex with HIV + partner in last 72 hours - Select Yes if applicable to the patient. If Yes, in the # of hours box, enter the number of hours.
- # Days Since Sex with HIV + Partner - In the box, enter the number of days.
- For each of the following items, select Yes if applicable to the patient.
- Sex with IDU
- Sex with anonymous partner
- Ever had sex with partner met on social media
- Sex with Commercial Sex Worker
- For the Sex With MSM (women only) item, select the time period that applies to the patient.
- Under Sex Outside US, for the Had sex outside continental US (past month) item, select Yes if applicable to the patient. If Yes, in the Locations field, enter the locations.
- Under Sex with Foreign Partner, for the Had sex with foreign partner (past month) item, select Yes if applicable to the patient. If Yes, in the Nationality field, enter the nationality for each partner.
- On the toolbar above the menu, click the Save icon
