Confirmatory Disclosure - STD module

In the STD module, the Confirmatory Disclosure form is where you record the disclosure of test results that confirm the preliminary positive HIV test result.

On the main menu navigate to HIV > Confirm. Dis.

Document a confirmatory disclosure

  1. In the OAIDS Confirmatory Specimen Number box, enter the specimen number for the test that confirmed the preliminary positive test result.
  2. For the Patient agrees OAIDS confirmatory specimen number matches OAIDS on specimen item, select Yes if the numbers match.
  3. In the Final Result box, click the arrow and select the type of result.
  4. For the Confirmatory disclosure conducted item, select Yes if applicable to the patient. If Yes is selected, in the Confirmatory disclosure held by box, click the arrow and select the staff member.
  5. Under Partner Services, select the check box for each item that applies to the patient.
  6. Under Referrals Made, select the check box for each organization or program the patient is being referred to. 
  7. For the Consent obtained to contact medical provider item, select Yes if the patient consented. 
  8. For the If patient no-show, field record cut item, select Yes if applicable to the patient.
  9. In the Outcome box, click the arrow and select the outcome.
  10. In the Provider Name box, enter the name of the provider supplying the confirmatory disclosure information.
  11. For the Crisis counseling needs assessed item, select Yes if the patient's needs were assessed. 
  12. On the toolbar above the menu, click the Save icon .
