Consents - STD module

In the STD module, on the Consents form you can view, add, and print patient consents for the release of medical information and for treatment.

On the main menu click Consents.

Consent history

  • To import a consent document into the patient's record, it must be in PDF format.
  • To view a consent document, in the grid click the arrow to the right of the consent date. The document opens in a separate browser tab.

Add a consent

A red star indicates a required field. Required fields must be completed before submission.

  1. Below the Consent History panel, click Add Consent.
  2. In the Form Name box, click the arrow and select the choice that best describes the type of consent. This is required.
  3. In the Document box, click Browse to select a document from the file system. This is required.
  4. In the File Upload window, select the file and click Open.
  5. In the Name box, enter the name that will display for the consent document. This is required.
  6. In the Description box, optionally enter a description for the consent.

Note: To clear the entries and start over, click Reset. To discard the entry, click Cancel.

  1. When finished, click Upload. The uploaded consent is added to the Consent History grid.

Print a consent

  1. In the Consent History grid, for each consent to be printed, select the check box to the left of the document name. Or click Select All to select all consents. To clear all selections, click Clear All.
  2. Click Print Consents. This opens a new browser tab for each consent selected.
  3. Move the cursor over the bottom of the consent to display the taskbar.
  4. In the taskbar, click the printer icon.
  5. In the Print dialog, select options and click Print
  6. To return to the Consent History, close the browser tab for each consent.
