Functional Assessment Record

QI records are required for the state of Michigan to process clients in mental health or dual diagnosis programs.

Details

QI functional assessment records:

  • can be entered and edited in either ProviderConnect and in the Functional Assessment Tools form (Avatar PM).

  • filed in ProviderConnect are sent to Avatar MSO, and from Avatar MSO to the Michigan state agency.

  • record information is managed in ProviderConnect by the contracting provider. QI information is display-only for ProviderConnect administrators.

Steps

  1. In the Main Menu click Lookup Client.

  • The Search Criteria screen displays.
  1. Enter the client's name, ID, social security number.

  2. Click Search By Criteria.

  • The Search Results screen displays.
  1. Click a client ID number.

  • The ProviderConnect Demographic screen displays for the client and agency.
  1. In the Client Sidebar menu click QI Forms.

  • The QI Forms screen displays.

Create a Functional Assessment Record

  1. In the Functional Assessment section click Add Functional Assessment Record.

  • The Functional Assessment Information screen displays.
  1. Enter the functional assessment date in the format m/d/yyyy.

  • The current date populates and can be edited.
  1. Select the interval type.

  2. Select the functional tool used to capture the assessment data (CAFAS - Child and Adolescent Functional Assessment Scale, or DD Proxy Measures).

  1. Apply selections as appropriate for the functional record.

  2. Click Save Record.

  • The functional assessment record displays in the Functional Assessment section.

View or Edit a Functional Assessment Record

  1. In the Functional Assessment section, click a record date.

  • The client's functional assessment information shows display only.
  1. Click Edit Record.
  • The screen can be edited. Edit selections as appropriate, click Save Record.

Generate the Audit Log Report to view information traffic sent from ProviderConnect to Avatar MSO.
