Coverage Information Section
- In the Eligibility Verified field, select Yes if the client's eligibility has been verified.
- In the Coverage Effective Date field, enter the first day of coverage.
- In the Date Benefits Denied field, enter the coverage denial date if applicable.
- In the Coordination Of Benefits field, select whether benefits are coordinated between the subscriber's guarantors.
- If appropriate, select the coverage denial code.
- Enter notes as appropriate.
