Client Profile

QI records are required for the state of Michigan to process clients in mental health or dual diagnosis programs.

Details

QI client profile records:

  • can be entered and edited in either ProviderConnect or in the Michigan Client Profile form (Avatar PM).

  • filed in ProviderConnect are sent to Avatar PM, and from Avatar PM to the Michigan state agency.

  • record information is managed in ProviderConnect by the contracting provider. QI information is display-only for ProviderConnect administrators.

  1. In the Main Menu click Lookup Client.

  • The Search Criteria screen displays.
  1. Enter the client's name, ID, social security number.

  2. Click Search By Criteria.

  • The Search Results screen displays.
  1. Click a client ID number.

  • The ProviderConnect Demographic screen displays for the client and agency.
  1. In the Client Sidebar menu click QI Forms.

  • The QI Forms screen displays.

Create a Client Profile

  1. In the Client Profile section click Add Client Profile.

  • The Client Profile Information screen displays.
  1. Enter the client profile date.

  • Enter dates in the format m/d/yyyy.
  1. Select the following:

  1. Apply other selections as appropriate.

  2. Select whether the client will be excluded form Michigan QI reporting.

  3. Select whether the client is covered by Medicaid.

  • If Yes is selected, enter the client's Medicaid number.
  1. Select whether the client is Hispanic or Latino.

  2. Select the client's race.

  3. Click Save Record.

  4. The client profile record displays in the Client Profile section.

View or Edit a Client Profile Record

  1. In the Client Profile section, click a record date.

  • The client's profile information shows display only.
  1. Click Edit Record.

  • The screen can be edited.
  • Edit selections as appropriate, click Save Record.

Generate the Audit Log Report to view records sent between ProviderConnect and Avatar MSO.
