License Information - PM

Record practitioner license information.


  1. Go to: Avatar PM > Practitioner > Practitioner Information.
  2. In the Select Staff screen, enter the staff name or ID, and select. Click Select.
    • If the practitioner has previous license records, select the record. Click Edit to change information, or click Add to create a new record.
  3. In the State Licensed field, select the state.
  4. In the License Number field, enter the information.
  5. In the Registry Number field, enter the practitioner's registry number, if provided by the licensing state for their registry.
  6. In the Type Of License field, enter the type of license.
  7. in the License Issuance Date field, enter the date the license was issued.
  8. In the License Expiration Date field, enter the license expiration date.
  9. Click Submit.
