Fast Financial Eligibility - PM

Enter minimal client financial eligibility information for new admissions, and for clients whose guarantors and benefit plans do not need to be customized.

Benefit plans are customized for a client in the Financial Eligibility form.

  • If financial eligibility information was filed for the client in the Financial Eligibility form, that information populates this form and can be edited.
  • When this form is filed, guarantor, client demographic, and plan information populate the Financial Eligibility form.

  1. Go to: Avatar PM > Client Management > Account Management > Fast Financial Eligibility.
  2. In the Select Client screen, enter the client name or ID, and choose Select.
    If the client has multiple episodes, the Episode Information pre-display shows.
  3. Select the episode and choose OK.
  4. In the Guarantor #1 field, search and select the appropriate guarantor. 
    Note: This will enable and require the Plan #1, Policy #1, Eligibility Verified #1, and Client's Relationship field and will enable Guarantor #2 field.
  5. In the Plan #1 field, select the appropriate plan from the drop-down menu that correlates to Guarantor #1.
    Note: This list contains all benefit plans with the same guarantor nature (Guarantors/Payors form)
  6. In the Policy #1 field, enter the policy number that correlates to Guarantor #1 and Plan #1.
  7. In the Eligibility Verified #1 field, make the appropriate selection based on the client's eligibility verified for Guarantor #1, Plan #1, and Policy #1.
  8. In the Client Relationship to Subscriber #1 field, make the appropriate selection of the client's relationship to the subscriber of Guarantor #1, Plan #1, and Policy #1
    Note: If Self is selected, the client's name and birth date of the individual into Subscriber's Name #1 and Subscriber's Birth Date #1 fields. 
  9. In the Subscriber's Name #1 field, enter the subscriber's name. 
  10. In the Subscriber's Birth Date #1 field, enter the subscriber's date of birth.
  11. If appropriate, repeat steps 3 - 9 for each additional guarantor.
  12. Once finished, select Submit.
