- If Network, select which one in the Network field. This field is populated by the Refresh Network List button on the CareConnect CCD Upload Configuration form.
Note: If the HIE configured in previous steps cannot be found, return to the Configuration form and click Refresh Network List. - If Organization, search for and select which one in the Organization field.
- If Network, select which one in the Network field. This field is populated by the Refresh Network List button on the CareConnect CCD Upload Configuration form.
- In the Consent to Query field, select whether the consent for the organization to access the client's health information is to be granted, revoked, or only permitted for emergencies (if Emergency Only, consent may or may not be accessible in myAvatar).
- This field is enabled when the Consent For field is set to Organization, and when the Consent Required For field on the CareConnect CCD Upload Configuration form is set to Consent to Query.
- This field is also enabled when the Consent For field is set to a Network with Carequality in it.
- When this form is filed, information will be transmitted to CareConnect.

- Some organizations may have a policy in place whereby consent must be renewed each year. In that scenario they may choose to enter an end date when granting consent, though an end date is not required. In the instance that no end date is required, consent will remain in effect until revoked.
- If consent is being revoked, end date will not be enabled.
