Create a Practitioner - PM

How to create and maintain contracted provider enrollments within myAvatar.
Prerequisite: Sufficient access to the Practitioner Enrollment form.

  1. Go to: Avatar PM > Practitioner > Practitioner Registration > Practitioner Enrollment 
  2. Enter the name of the practitioner.
  3. Select New Staff.
  4. Complete all required fields (those marked in red) and as many other fields as possible.
    Note: The more completely you fill out this form the more time you will save in the future.
  5. Search to see if there is an existing Performing Provider Registration (MSO) record using the the Performing Provider search.
    Note: If none are found, leave the “Performing Provider” and “Performing Provider Registration” fields blank. Leaving these fields blank is the trigger to create a new and matching Performing Provider Registration (MSO) record when the Practitioner Enrollment (PM) form is submitted for this record.
  6. Complete the Categories/Taxonomy section of the Practitioner Enrollment (PM) form.
    Note: This information will be utilized to create the performing provider’s Primary License Type
    After submitting the form, you can re-launch Practitioner Enrollment (PM) to see that the Performing Provider and Performing Provider Registration fields are now completed and have the Performing Provider (MSO) record linked.
    Once the practitioner to performing provider link has been made, it cannot be edited.
  7. Launch the Performing Provider Registration (MSO) form and search for the performing provider by name or unique ID assigned to the staff in Practitioner Enrollment (PM). When creating practitioners through this process, myAvatar will assign the same unique ID in both Practitioner Enrollment (PM) and Performing Provider Registration (MSO) if the IDs are available.
  8. Open the record to review what information was created with this record. You will be able to make any updates and complete the Performing Provider Registration (MSO) record to document any missing fields.
