Order Consent
Record a client consent for particular order codes.

- Order codes can be procedures, tasks, tests, and medications.
Prerequisites:
- The codes to provide consent for must be set up as requiring consent through the Require Consent field on the Order Code Setup form.
- This form is added through myAvatar OE 2017 Update 15.
- The Consent Date automatically defaults to today's date, but can be changed.
- In the Last Review Date field, enter the date of the last time this consent was reviewed.
- In the Order Code Search field, enter the name of the code and click Search.
- The Order Code(s) field populates with all of the codes that meet the search criteria and are set up to require consent. Select all relevant codes and press TAB to exit the field.
- The Selected Order Code(s) field displays information for all of the codes checked in the Order Code(s) field.
- If a note or description was entered in the Consent Text field (Order Code Setup form), this information displays along with the drug name.
- If no information was entered in the Consent Text field, the generic consent text description displays: "Client has read a medication consent form and understands his/her rights to refuse this medication, and that it cannot be administered to him/her until he/she has spoken with his/her physician and given consent to it, except in an emergency."
- In the Consent Override Reason field, select the consent override reason code for why an order code is being chosen.
- In the Comments field, enter any additional information to accompany the consent.
- In the Client's Signature field, click Sign. The Please Sign dialog box launches. Have the client sign their signature on while portion of the window, or with a signature pad.
- Click Clear to delete the signature and try again.
- Click OK to save the signature. An image of this signature will display in the field.
- Click Cancel to exit without a signature.
- Click Submit to file the consent record.
