Clinical Reconciliation and Order Entry Medication Orders

Reconcile an myAvatar Order Entry medication order by performing one of these actions: create a new order, continue an order, create a new medication order and discontinue/inactivate an existing order, defer an order until the next reconciliation session, or discontinue/inactivate an order. 

Avatar Order Entry history-only orders are considered active home medication orders. These orders can be reconciled by either continuing the order without making changes or creating a new order for the medication item. 

Avatar Order Entry prescribers are able to create new orders or discontinue orders through the Clinical Reconciliation form in myAvatar CWS. When creating new orders, checks are done for age/problem, duplicate order, non-formulary medications, and First Databank medication.
