Provider Search Section

You can use myAvatar MSO to find a provider in many MSO forms. When you find a provider, their information goes to the Provider to be Authorized field. This field is in the Service Authorization section. When you enter information, you see only the reports you need.


  1. In the Search Effective Date field, enter the date.
  2. In the Office Address (Zip Code) field, enter the ZIP code.
  3. In the Specialties (SPE) field, select specialties.
  4. In the Other Languages (LAN) field, select languages other than English.
  5. In the License Type (LIC) field, select provider licenses.
  6. In the Performing Provider Race/Ethnicity (ETH) field, select provider ethnicity.
  7. In the Treatment Population Age Group (POP) field, select the provider treatment age group.
  8. In the Performing Provider Gender (GEN) field, select the provider gender.
  9. In the Performing Provider Age Group (AGE) field, select the provider's own age group.
  10. In the Special Accommodations (ACC) field, select required special accommodations.
  11. In the Handicapped Accessible (HAN) field, select Yes if the provider has handicap accessibility.
  12. In the Hospital Privileges (PRV) field, select the provider hospital privileges. Defined in the Hospital Privileges field (Contracting Provider Registration form).
  13. In the Find In Network/Out Of Network Or Both field:
  • Select Both to include in and out of network providers.
  • Select In Network to include providers in network.
  • Select Out Of Network to include providers out of network.
  • This is defined in the In Network/Out Of Network field (Contracting Provider Registration form).
  1. In the Funding Source field, enter the funding source name, and select.
  2. In the Minimum Number Of Matching Items For Selection field, enter the lowest number of matching search criteria for a match to display.
  3. Select Display Search Results to display the Provider Search screen.
  4. Select the provider then, Ok.
