Member Termination

Remove a member's association with a funding source and benefit plan.

  • Services posted beyond this date for this member and funding source will be denied unless Yes is selected in the Member Ineligible on Date of Service field of the Approve/Pend/Deny Rules Definition form.

  1. In the Select Client screen, search for and select the client by name or ID.

  • Choose the client, click Ok.
  • A pre-display shows the member's enrollment history.
  • Select an enrollment row, click Edit.
  1. The Funding Source field displays the member's funding source.

  2. The Plan field displays the assigned benefit plan for the enrollment.

  3. The Effective Date Of Enrollment field displays the effective date of the funding source enrollment.

  4. In the Termination Date Of Enrollment field, enter the termination date.

  5. In the Reason for Termination field, select the termination reason.

  6. In the Termination Comments field, enter comments to include with the record.

  7. Click Submit.
