Enter member demographics.

  • If the member is currently a client in myAvatar PM, client demographics populate and can be edited.

Avatar MSO > Membership Management > Member Termination

  • If the member is not a client in PM, an admission is created in the parent system when this form is submitted, if:
    Yes is selected in the File Admission on Member Enrollment field (MSO to Parent System Integration Mapping form).
    Member demographics entered in this form will be filed in myAvatar PM.
    If No is selected in the File Admission on Member Enrollment field (MSO to Parent System Integration Mapping form),
    member demographics are filed in myAvatar PM as a “shell of a PM client record,” and are not recorded as an admission.

  • If the member is deleted from myAvatar MSO before an admission is created, the “shell of a PM client record” is also deleted.

  • Enter member demographic information as appropriate.
