Discharge Wound

myAvatar Flowsheet allows for the documentation of wounds upon discharge. Action is needed when a client/patient is being discharged and the wound has not healed.   

  1. Sign into myAvatar.
  2. Select a client/patient.
  3. Select an episode.
  4. Select the Flowsheet console.
  5. Select the Wound Care collection.
  6. Select a wound from the wound list.
  7. Select Actions.
  8. Select Discharge Wound.
  9. Confirm a final assessment has been completed with Yes or No.
    • When Yes is selected:
      • The Reason for Discharging modal displays.
      • Enter reason and Save.
      • The wound becomes inactive and is no longer present in the active list.
    • When No is selected, myAvatar allows for assessment documentation.
      • Enter wound care assessment and Sign.
      • To discharge the wound, repeat steps 1-9, then select Yes.

Results: 

  • The discharged wound is listed when Include Inactive Wounds is selected.
  • All previous assessments and associated images can be viewed, printed or downloaded.
