Clinical Pathways tied to an assessment
Clinical Pathways can be configured to your modeled assessments to drive the enrollment at the time of the assessment.
Clinical Pathways can be added to two product forms:
The Columbia Suicide Risk Assessment was delivered as a product form with myAvatar CWS 2020 Update 16.1. This form is derived from the Columbia Suicide Severity Rating Scale tools. Agencies have often modeled their own but if they have not, they can use the product versions. On both The Columbia Suicide Risk Assessment and Columbia Suicide Risk Assessment since last visit, the Clinical Pathway section will become enabled once “Final” is selected.
The first option selection (Enroll in Clinical Pathway) offers a Yes or No.
- If Yes is selected, Pathway Name will become enabled and required. If the client is already enrolled in the same selected pathway, the system will display an error stating the client is already enrolled.
- If No is selected, Reason will become enabled and required. This is for the clinician to provide a reason for why they may not be enrolling a client in a pathway at this time.

- Patient Health Questionnaire (PHQ-9)

- Vision and Hearing Screening

- Other user or agency-modeled (customized) assessments
