MHSA - Key Event Tracking Form - Cal-PM
Collect Mental Health Services Act (MHSA) key event data, such as changes in living conditions, employment or education.
- The Key Event Tracking Form (KET) can be used to indicate Key Event Changes for the patient (address change, educational status change, etc.), or to discontinue or re-establish a partnership between the state and client. When a partnership is discontinued, a KET can be used to re-establish the partnership if the re-establishment date is less than a year from the discontinue date. If the re-establishment date is more than one year from the discontinue date, a new Partnership Assessment Form (PAF) would be required to establish a new partnership with the state.
- Age categories:
- Children and Youth, aged 0-15
- Transitional Age Youth (TAY), aged 16-25
- Adults, aged 26-59
- Older Adults, aged 60 and older
- Deleting a Key Event Tracking record will also clear associated GUID values that were created within the MHSA XML Submission form.
Important: A KET that contains a partnership status change cannot be edited or deleted. If the key event changes includes a discontinuation in the partnership status, then other information about the partnership should be entered on a separate KET prior to submitting the KET for the discontinuation. Once the partner is discontinued, the partner becomes inactive and no new KETs can be made for the partner.
- The Partnership Assessment Form must be filed with Final status (Assessment Status field) for the client.

Go to: Avatar PM > Client Management > California Data Reporting > MHSA Assessments > Key Event Tracking Form -
In the Select Client screen, enter the client ID in the Client Name/ID field, select the Search button, and then the corresponding entry.
- If the client has multiple episodes, the Episode Selection pre-display shows. Select the client episode, select Edit.
- If the client has previous key event tracking records, the pre-display shows. Select the assessment, select Edit.

In the Date Completed field, enter the key event tracking date. This date is used to calculate the partner’s age.

The Partnership Form Type field shows the partner's age category based on the Date Completed field and the client's DOB.

In the Assessment Status field:
- Select Draft to create a key event tracking record that is in process. When the assessment is completed the status can be changed to Final.
- Select Final for a key event tracking record that has been completed. This enables the KET to be picked up in the compile. If required, Final status can be changed to draft in the MHSA Assessments Draft/Final Override form.
Note: Records marked as Final cannot be deleted.
