Widget - Service Authorization Information
Displays all member service authorization records (active and inactive) in the system for the selected client. Records appear in descending order by Auth End Date and Auth Number.
The Auth Number serves as a link to open the Service Authorization form for each record listed. Only users with access to the form will be able to open this link.
Service authorization records containing more than one CPT/Revenue Code will be displayed repeatedly in widget results, repeated once for each different code included in Service Authorization.

- Benefit Plan
- Funding Source
- Provider
- Level of Care
- Status
- Procedure
- Requested Units
- Approved Units
- Claimed Units
- Liability
