Psychosocial History Assessment

This form allows you to record physical, emotional, behavioral, social, and legal issues for a client.

  1. Go to: Avatar CWS > Assessments > User Defined Assessments
  2. After selecting the form, search for the Client Name/ID in the Select Client screen. Select the client's name. 
  3. If the client has multiple episodes, select the appropriate episode. 
  4. A pre-display will appear if a record has been previously filed for the client and episode. Otherwise, the form will open. 
  5. In the Assessing Clinician field, search for the clinician by typing their ID number or their name in Last Name,First Name format. Click the search button and select the clinician. 
  6. In the Assessing Date field, enter the assessment date by entering it in MM/DD/YYYY format, using the calendar icon, or by using the T or Y buttons for today or yesterday. 
  7. In the Case Manager field, select whether or not there is a case manager assigned.
    Note: If Yes, enter the Case Manager Name.
  8. In the Contact One Name field, enter the contact's name in Last Name,First Name format.
  9. In the Contact One Relationship field, select a relationship option. 
  10. The following fields in the Community Supports/Agencies section are set up in the RADplus Dictionary Update form (myAvatar CWS):​
  11. In the Contact One Street Address One field, enter the address. 
  12. In the Contact One City field, enter the contact's city. 
  13. In the Contact One State field, select the state. 
  14. In the Contact One Zip Code field, enter the zip code in 5- or 9-digit format. 
  15. In the Contact One Phone field, enter a phone number in a 10-digit format. 
  16. Repeat steps 8-15 for Contact Two and Contact Three. 
  17. In the Reason for Admission / Precipitating Stressors field, enter any information about the reasons and stressors preceding this assessment.
  18. In the Income / Financial Supports field, select all of the sources of income that apply.
  19. Enter any sources not listed in the If Other Income / Financial Supports Specify field if Other was selected.
  20. In the Financial Needs field, describe any financial needs the client has.
  21. In the Current Living Arrangements and Support System field, enter any relevant information about the client's current living situation.
  22. Navigate to the next sections. 
  23. Once all required fields are filled out, click Submit.
