Physician Assessment

Record the client's medical history, psychiatric history, and physical assessment.

  1. Go to: Avatar CWS > Assessments > User Defined Assessments > Physician Assessment.
  2. In the Select Client screen, search for and select the client ID in the Client Name/ID field. 
  3. If the client has multiple episodes, choose the episode; select OK.
  4. Enter the clinician's name in the Assessing Clinician field; select Process Search. Choose the clinician's name; select OK.
  5. Enter the Assessing Date in MMDDYYYY format, or select the calendar and choose a date, or select T for 'today' or Y for 'yesterday'.
  6. Enter the Assessment Time in the HH:MM AM/PM format, or select Current Time. 
  7. Enter the Informants (Identity) who provided information.
  8. Enter the Referral/Admission Circumstances for the client's admission or referral. 
  9. Enter the client's Chief Complaint. 
  10. Enter the client's History of Present Illness. 
  11. Select Medical History and complete the client's medical history, including allergies, illnesses, medications, and family medical history. 
  12. Select Psychiatric History and complete the client's psychiatric history, including alcohol and substance abuse, family psychiatric history, and personal/social history.
  13. Select Physician Assessment and complete the client's physical assessment, including vital signs and neurological and movement information.
  14. Select Submit.
