Nutrition Screening

Record diet considerations for developing a client's nutrition plan.

  1. Go to: Avatar CWS > Assessments > User Defined Assessments > Nutrition Screening.
  2. Select the correct client and episode.
  3. In the Assessing Clinician field, enter the clinician's name, search for and select the clinician.
  4. In the Assessing Date field, enter the assessment date.
  5. In the Do You Follow a Special Diet field, select whether or not the client observes any dietary restrictions.
    • If Yes, describe the details of the special diet in the Special Diet Comments field.
  6. In the Do You Have One or More of These Medical Problems field, select the appropriate medical problems.
  7. In Foods or Drinks Omitted field, select whether or not the diet includes omitting certain foods or drinks.
    • If Yes, describe the omitted items in the Foods or Drinks Omitted Comments field.
  8. In the Foods or Drinks Omitted in Diet Due to field, select the reasons that the client omits certain foods or drinks. 
  9. In the Do You Take Vitamin or Mineral Supplements? field, select whether or not the client takes any supplements.
    • If Yes, describe the supplements in the Vitamin or Mineral Supplements Comments field.
  10. In the Do You Use Alcohol field, select whether or not the client drinks alcoholic beverages.
    • If Yes, describe the usage in the Alcohol Comments field.
  11. In the Do You Use Tobacco field, select whether or not the client takes tobacco in any form.
    • If Yes, describe the usage in the Tobacco Comments field.
  12. In the Do You Use Caffeine field, select whether or not the client takes caffeine in any form.
    • If Yes, describe the usage in the Caffeine Comments field.
  13. In the Do You Use Other Substances field, select whether or not the client uses any additional substances.
    • If Yes, describe the substances in the Other Substances Comments field.
  14. Enter any additional information in the Additional Comments field.
  15. Once finished, select Submit.
