Health and Review of Systems

Document the client's current health. Functionality can be added to this form through Site Specific Section Modeling.

  1. Go to: Avatar CWS > Assessments > Product Assessments
  2. In the Select Client screen, enter the client ID in the Client Name/ID field, click Select Client Name/ID Search. Select OK.
    • If the client has multiple episodes, select the client's episode, click OK.
    • If the client has previous heath and review of system records, a pre-display screen displays. Select the record, click Edit. Or, to add a new record, select Add
  3. In the Assessing Date field, enter the date.
  4. In the Duration of Visit (Min) field, select the length of the client visit.
    This amount can affect the service duration, for services that are duration based.
  5. In the Draft/Final field, select Draft if the information is in process.
    Select Final if the information has been finalized.
  6. In the Type of Client field, select New or Established.
  7. In the Chief Complaint field, enter a description of the client's main physical complaint.
  8. Enter appropriate responses In the Location, Quality, Severity, Duration, TimingContext, Modifying FactorsAssociated Signs and Symptoms, Past History, Family HistorySocial History, and Comments fields. 
  9. In the Include In Syndromic Reporting field, select Yes or No
  10. In the Diagnosis 1 field, type a diagnosis in the search field and press the Search icon. Select the diagnosis from the results. 
  11. In the Part Of A Multiple Birth field, select Yes or No.
    Note: If Yes is selected, enter the order in the Birth Order field. 
  12. Complete the Review of Systems section. 
  13. When finished, click Submit.
