Forensic Assessment

Record client psychological evidence for judicial purposes.

  1. Go to: Avatar CWS > myAvatar CWS Assessments > User Defined Assessments
  2. In the Select Client screen:
    1. Enter the Client ID of Client Name in the client search field, the results will dynamically display.
    2. Select the matching client from the results and the Select Episode screen or Assessment will display.
  3. If the client has multiple episodes, in the Select Episode screen, select the client's episode from the table.
  4. In the Assessing Clinician field, enter and select the clinician's name from the matching results dropdown.
  5. In the Assessing Date field, enter the assessment date.
  6. In the Statute field, select the client's statute.
  7. In the Date of Staffing field, enter the staffing date.
  8. In the Staff Present field, select the staff members present at the assessment.
  9. Enter client interview information in the following fields:
    • Interview Findings
    • Summary of Recommendations
    • Diagnostic Impression
  10. Click Submit.
