In the Date field, enter the exam date. The current date populates, and can be edited.
In the Tooth Number field, enter the tooth number, or click the tooth. Multiple teeth can be selected.
In the Tooth Surface field, select the surface of the teeth. If only one tooth number is selected, then multiple values can be selected in the tooth.
In the Treatment Type field, select the treatment type.
In the Tooth Status field, select the tooth status. Multiple values can be selected.
In the Treatment Notes field, enter any associated notes.
In the Treatment Completed field, enter the completion date. If a value is entered in this field and this form is opened again, this field will display in blue and cannot be edited.
