Adjunctive Therapy
Enter client information for therapy that is given in conjunction with the client's primary treatment.
- Go to: Avatar CWS > Assessments > User Defined Assessments
- Select the correct client and episode.
- In the Assessing Clinician field, enter the clinician's name, then select Process Search. Select Ok.
- In the Assessing Date field, enter the assessment date.
- In the Additional Dates field, enter any additional dates.
- In the Form Type field, select whether the assessment is for adjunctive therapy screening or assessment.
- In the Interview Information Obtained From field, select the interview information sources.
- In the Comments field, enter any associated comments.
- In the Client's Past Leisure Interests field, enter any past leisure activities.
- In the Client's Current Leisure Interests field, enter any current leisure activities.
- Complete the Daily Activities subsection of the form by adding Typical Day Spent Doing, Daily Living Skills Responsible For, and Community Resources Utilized information.
- Complete the Resources subsection of the form by adding Resource Needs Related to Leisure Involvement, Reads, and Writes information.
- Complete the Behaviors subsection of the form by adding Changes Client Would like to Make in Use of Time, Observations, Behaviors/Affect, Comments, and Cognitive Ability information.
- Complete the Strengths/Weaknesses subsection of the form by adding Strengths, Weaknesses, Activities Supporting Addictive Disease and Activities as Alternatives to Addictive Disease information.
- Complete the Conclusions subsection of the form by adding Conclusions, Recommendations/Comments, Adjunctive Therapy Recommendations, Discharge Issues/Recommendations Related to Leisure ,and Additional Assessment Needed this Time information.
- In the Assessment(s) field, select recommended assessments.
- Select Submit.
