Fast Dose Dispensing

About the Fast Dose Dispense Form

Fast Dose Dispensing is where medication dispensing is tracked. It displays pertinent information within the initial form including the client picture, outstanding take-out bottles, dispensing history, Sticky Notes and active Care Plans. It also allows for the extension of Take Home doses. 

From this form, you can access an array of other functions available in the Avatar AM system. To access any of the forms listed below Fast Dose, you must select the client ID you wish to access.This form will allow you to clear Care Plans, view client alerts, extend take out bottles, collect take out bottles, return Inventory or track spills, and enter exceptions (in the case of something like a power outage where manual dispensing was required). 

Once the Dispense button is selected, a second form displays that controls the actual dispensing. There are a few alerts that can be configured such as client not checked in, no effective medication order, and active Care Plan alerts. 


The dispensing form displays information such as medication order information, clinic/take home schedule for the week, amount of medication ready to be dispensed, date of dispense, missed doses, etc. It also displays the inventory that is opened and the current inventory level.  Once the medication has been dispensed, the user is returned to the main Fast Dose Dispensing form.
The form also incudes sections:

  • Care Plan - the Care Plan form is available for adding or resolving existing Care Plans.
  • Spill / RTI - the form allows the user to track if medication was spilled and therefore needs to be re-dispensed, or medication needs to be returned to inventory. For example a take-home dose was prepared, but the client did not show up at the clinic that day.
  • Exceptions - allows for medication exceptions to enter back-dated dosing due to say a power outage, or to give a supplemental dose based on a doctor's order.
  • Med Order Info - displays the history of medication orders for selection and the associated dispening schedule.
  • Medical Notes - allows for the review or entry of progress notes.
  • Medical Information - this form contains site specific fields that can be configured to gather and display pertinent medical information such as other items the client is being treated for by other practitioners (PCP, etc.).
  • Availability - the tracking of days when the client is unavailable for dispensing. This would include things like hospitalization, vacation and dispensing at an alternate clinic, etc. This is used so that they are not tracked as missing a dose on those days.
Prerequisites
  • Client must have a current medication order through Medication Order or Quick Medication Order.
  • Medication inventory must be activated through Pump and Inventory Management.
  • Depending on configuration, the client may need to have been checked-in through Front Desk Information.
  • Depending on configuration, the client may need to have any Care Plan Activities cleared either through Front Desk Information, Care Plans, or the Fast Dose Dispensing / Care Plan section.

Dispensing and Returning Take Out Bottles

When a client returns outstanding bottles, you will enter the return information in the Fast Dose form. If the client has outstanding take out bottles and is returning them, count the number of bottles the client is returning, make sure the number matches with the number listed in Number Of Take Out Bottles To Return field. Take Out Bottle is when a client checks out a bottle to dispense their own dosage.

Select a client. (The number of outstanding bottles, number of takeout bottles, and active care plans will automatically populate.)

If the number of take out bottles matches, click Return Take Out Bottles.

If the number of take out bottles does not match then adjust the Number Of Take Out Bottles To Return to the number of bottles the client is returning and then click Return Take Out Bottles.

NOTE: The number in the “To Return” field will automatically populate with the Number Of Outstanding Take Out Bottles.

Dispensing Medication

Note: If the client has active care plans, they may need to be taken before dispensing. 

To dispense medication for the selected client: 

  1. Click once on the Dispense button.
  2. To return to the original fast dose dispense screen, select the Return button.
  3. When the client has been provided with all of the doses to be dispensed, the care giver will see “No More Dispenses”

Spill/RTI
Adjust inventory based on a dose that has been spilled and discarded or not taken by a client and returned to inventory.
Exceptions
Administer exception doses to a client based on orders by a physician.  Exceptions are ordered in addition to a daily dose, not instead of. For example, if a client comes in and the physician see them and determines that the client will receive an extra 10mgs of methadone based on his or her condition, the care giver will administer the original dose and then an exception dose of 10 mgs. This task requires a user password to complete.
Med Order Info
List the dispensing schedule for medical orders including dates, medication type and mount, and whether the dose if a clinic visit or takeout.
Medical Notes
Used to add a new medical note, or view previous medical notes. This section contains several subsections.
Medical Information
Lists other medications the client may be on, medical conditions, vaccinations, and allergies. Also allows the user to add medical comments.
Availability
Add or edit a client’s availability based on episode. May be used if a client is approved to go out of town and these dates need to be noted.  In this section, the start date, end date, and reason along with any additional comments will be added.
