Exception Dosing

Backdated Dosing (now called Exception Dosing) is used to either replace a dose that was unable to be recorded (due to a power outage or some other reason that manual dispensing had to occur) or to give a supplemental dose. A supplemental dose is normally given if the doctor requires a higher dose, but the client has already been dosed for the day. Even though the bottles you dosed from might physically be empty, myAvatar still thinks those bottles have methadone in them until exception dispensing is complete. If there is not a missed dose under the client’s record, myAvatar will not allow you to dispense an exception dose for that client.

Note:

AM Facility Defaults > Exception Dosing - This field indicates whether the Exception Dosing will allow for "Replace Ordered Dose" - meaning that the dose was recorded (normally because of a power outage), or "Independent Of Ordered Dose" meaning that a supplemental dose is being given (normally because a doctor has ordered an increase in the medication dosage after the dosing has already taken place).

Exception Dispensing (Same Day - client not showing as missed yet)


Example: had to hand dose for a few minutes due to power outage. Now you have to back dose in myAvatar so the client records and inventory reflect what was dispensed during the downtime.

  1. Close the inventory that is connected to the pump.
  2. Open inventory on unit 2, 3, or 4. Be sure that no methadone is open in unit 1 (should be the bottle from which you actually hand dispense).
  3. Using Fast Dose Dispensing, dispense each client as you normally would.

Exception Dispensing (Later Date - client showing missed dose(s))


Example: had a power outage on Monday & were not able to back dose on Monday. On the first day that the power is restored, you have to back dose in myAvatar so the client records and inventory reflect what was dispensed during the downtime.

  1. Close the inventory that is connected to the pump.
  2. Open inventory on unit 2, 3, or 4.
    Note: Be sure that no methadone is open in unit 1 (should be the bottle from which you actually hand dispense).
  3. Open Backdating Transactions Setup.
  •  Transaction date - this is the date of the hand dosing
  •  Effective date – this is the day you’re back dosing (today)
  1. Open Fast Dose Dispensing.
  2. Select your first client to back dose.
  3. Change the dispense date to the date that the hand dose was given.
  4. Dispense the client’s dose(s).
