Care Plan Activity Rules Definition
Care Plan Activity Rules Definition allows for a Care Plan to be invoked based on a particular scenario. The scenario can be associated with missed doses or another care plan result.
For example if a client misses two doses the application would assign a counseling session. Or, if a client had a positive drug screen, the application would assign a drug screen for the next two clinic visits.
This form is used to create Care Plan Activity Rules. Care Plan Activities are alerts and reminders of tasks that need to be completed. Those tasks could include drug screenings, pregnancy tests, or visits with a counselor.
Using this form, you can define a Care Plan Activity (CPA) that can be triggered at the time of client check in for a visit, a by a missed dose, by another CPA, or at a random point in time.
Note: Care Plan Activity Maintenance items must be added prior to the rules definitions.
Add or Edit a Care Plan Activity Rule
- Navigate to Care Plan Activity Rules Definition.
- Select Add.
- In the Care Plan Activity Rule Code field, create a code for your new rule.
Example: The code “TBSKIN” or “1” could be used for a TB Skin Test Care Plan Activity Rule. - Select Edit.
- Enter the name of the Care Plan Activity Rule into the Care Plan Activity Rule Description field.
Note: The Active field will default to Yes when creating a new Care Plan Activity Rule. After submission, you can open this form again and switch the Active field to No to deactivate the rule. - Select the trigger for the rule from the Care Plan Rule Characteristics field.
Note: This indicates whether the rule is triggered by one or more missed doses or if it is triggered by a Care Plan Activity. - OPTIONAL:
- If Missed Dose is selected, type in how many missed doses in a row must occur before this rule is triggered in the Concurrent Missed Doses field.
- If Care Plan Activity is selected in the Care Plan Rule Characteristics field, the Care Plan Activity Taken field will be enabled. Here is where you will select the Care Plan Activity to trigger another Care Plan Activity (which we will select later in the form).
- In the Schedule Care Plan On Entry Of field, select whether the second Care Plan will be triggered when the first Care Plan is taken or when results are entered.
- If Results Date is selected, the Schedule Care Plan Rule field will become enabled. Should the results of the CPA trigger the rule always OR “only if care plan requires attention?” Results that require attention can be defined in Care Plan Activity Maintenance form.
- Indicate how many occurrences of the new care plan activity should occur within how many days (In the Schedules section).
- If the new care plan should happen once immediately, enter 1 in the Number of Occurrences and enter 0 in the In Number of Days field.
- In the Care Plan Activity To Be Scheduled field, select the care plan activity to be triggered from the missed doses or from the Care Plan Activity Taken.
- Click the Print Care Plan Activity Rules button to print a report of all active Care Plan Activity Rules.
- Click Submit.
