Leave Status Change - PM

Return a client from leave and assign a new leave status. This form combines the functionality of the Leaves and Return From Leaves forms.

Prerequisites:

  • The client must be enrolled in an inpatient treatment program.
  1. Go to: Avatar PM > Client Management > IP/Residential Management > Leave Status Change.
  2. In the Select Client screen, enter the client name or ID, search for and select the client.
    Result: The client's leave information populates the associated fields.
  3. In the Effective Date Of Leave Status Change field, enter the leave change date.
  4. In the Effective Time Of Leave Status Change field, enter the leave change time. The effective date and time are used for client leaves, and for return from leaves.
    Note: If the leave status change date is the same as the leave date, the time of the leave status change must be later than the leave time.
  5. The Current Type Of Leave From field shows the client’s current type of leave.
  6. The Unit From field shows the client’s current unit.
  7. In the Reason For Closure Of Leave field, select the reason for closing the client leave.
  8. In the Return Condition field, select the return condition.
  9. In the Return Condition Description field, enter the client’s condition.
  10. Select the Unit, Room and Bed from the appropriate fields.
  11. The Licensed/Unlicensed field shows the bed license status.
  12. In the Room and Board Billing Code and Daily Charge Code fields, select the appropriate codes.
  13. in the Type Of Leave From field, select the type of leave.
  14. For the Reason For Leave field, select the reason for the client’s leave.
  15. In the Leave Responsible Person field, enter the name of the person responsible for the client leave.
  16. In the Leave Location field, enter the client’s leave location.
  17. In the Leave Location Address field, enter the leave location address.
  18. For the Mode Of Transportation field, enter the client’s mode of transportation.
  19. In the Date Guardian Notified field, enter the date the guardian was notified.
  20. In the Notified By Whom? field, enter the name of the person notified of the client leave.
  21. In the Type Of Outside Service field, enter the type of service received for outside consultations.
  22. In the Leave Comments field, enter leave status change comments.
  23. Select Submit.
