Delete Family - PM

Remove a family record from the database. This form is generally used to remove a family that was entered in error.

Prerequisites:

  • A family must be created in the Family Registration form.
  • All family members must be deleted through the Family Registration form.
  • There should be no posted UMDAP services. UMDAP services must be deleted before the family can be deleted. Services are deleted in the Delete Service form. UMDAP services are rendered by a guarantor associated with UMDAP (Guarantors/Payors form).
  1. Go to: Avatar PM > Client Management > Client Information
  2. In the Select Family screen, enter the family name or ID, and select.
  3. Choose Select.
  4. Fields in this form are display only, and are defaulted from the Family Registration form.
  5. Select Submit.
