Client Consents - PM

Manage client consents for treatment and release of information.

  1. Go to: Avatar PM > Client Management > Client Information
  2. In the Select Client screen, enter the client name or ID, and select. Click Select. If the client has multiple episodes, the Episode Information pre-display shows. Select the episode and click OK.
  3. In the Exclude Client From Reports field, select Yes to exclude the client's consents from reports.
  4. To view a table of client consents, click Manage Consents.

Add a Consent

  1. Within the Manage Consents screen, click New Row.
  2. In the Consent Type field, double-click and select a consent type from the list. Click Select.  
  3. In the Status field, double-click and select a status from the list. Click Select.  
  4. In the Consent Date field, double-click and display a drop-down calendar. Double-click a date to select it.
  5. In the Reason for Consent field, double-click and select a consent reason from the list. Click Select.  
  6. In the Last Review Date field, double-click and display a drop-down calendar. Double-click a date to select it. This date is used to record when the consent was last reviewed. It is a CCHIT requirement for Advanced Directive consents.
  7. In the Signed Receipt on File field, double-click and select Yes or No to indicate if a signed receipt for the consent is on file. Click Select.
  8. In the Comments field, double-click and enter comments. Click Ok.
  9. Click Save.
  10. When finished, click Submit to finalize your changes.

Delete a Consent

  1. Within the Manage Consents screen, click anywhere in a row to select it.
  2. Click Delete Row.
  3. In the Confirm Delete dialog, click Yes.
  4. Click Save.
  5. When finished, click Submit to finalize your changes.

View a Consent

  • To view the client consents in Chart Review, click View Consents. The report generates automatically.

Print a Consent

  1. To print a client consent, in the Select Consent To Print field, select the consent.
  2. Click Print.
