About Guarantor Selection
The guarantor selection defines the financially responsible party for the client. You will need to repeat filling out this form as necessary until each of the client’s funding sources has been entered.
At first glance, the Guarantor Selection form can feel a little overwhelming. Although there are many data entry fields, many populate based on selections made in the form. For example, when you select the guarantor, the guarantor’s address, city, state, Zip code, and more populate. When the relationship to subscriber is self, the client’s name, address, Social Security number, and other information populate the form.
Individual responses or data entry are required for the Eligibility Verified, Effective Date, Subscriber Assignment Of Benefits, Subscriber Release Of Info, Subscriber Policy #, and Coordination Of Benefits fields.
