- Delay Reason Code (2300-CLM-20)

In the 'Diagnosis Code' fields, enter the diagnosis code.
- These values populate the associated 837 segments.

In the 'Procedure Modifier', and 'Diagnosis Reference' fields, enter information as appropriate.

- Facility Type Code (2300-CLM-05-1)
- Claim Frequency Code (2300-CLM-05-3)
- Release of Information Code (2300-CLM-09)
- In the remaining fields, enter information to populate the associated 837 loops and segments.
