Continuous Care report fields

The following table provides descriptions of the fields in the Continuous Care report in Homecare. 

Field Name

Description

Patient

Patient's name and ID from Patient > General > Basic.

Dates The dates that the patient had a continuous care status in Patient > General > Admissions & Status within the report. These are begin and end dates. 

Actual Nursing

The total of rounded Nursing hours identified as billable units delivered in the 24-hour day. These units are included in the calculation for the number of Continuous Care units on the claim.*

Actual Aide

The total of rounded Aide hours identified as billable units delivered in the 24-hour day. These units are included in the calculation for the number of Continuous Care units on the claim.*

Total Actual

The total of Actual Nursing + Actual Aide.*

Billing Rounding

The total of hours calculated using billing rounding.*

Meets Billing

Performing a raw calculation (including all Nursing and Aide hours in the day) indicates if the hours met the billing requirements. The services for this day must be evaluated against the clinical documentation to ensure all visit hours were related to the crisis.

It is essential to run this report before billing to make necessary changes to patient status (change from the Continuous Care level) or services (update certain services to non-billable, and so on) to ensure the reported data is correct.

Billing Exception Code

Provides a numeric exception code indicating why a particular day must be evaluated further to ensure accurate billing. Exception codes include:

  1. Total hours less than 8 hours
  2. Total hours reported exceed 24
  3. Nursing hours less than 50 percent of the total reported
  4. Hours were provided or were finished after the time of discharge

* Please note that the hours reported include all billable visits within the 24-hour day. This includes visits before the Continuous Care Level designation. Reviewing these hours and comparing them with clinical documentation is essential to confirm that each visit complies appropriately with the Continuous Care requirements.
