Begin Date

Specify the begin date to include only care plans that occurred within a certain date range.

End Date

Specify the end date to include only care plans that occurred within a certain date range.

Show Most Recent X Visit

To include only care plans that occurred during the patient's most recent visits, specify the number of recent visits you want to include.

Highlight Changes Since

Select the check box to highlight (underline) all changes since a certain date on this document.

Show Start and End Dates for Goals and Interventions

Select the check box to include the start and end dates in the descriptions of all goals and interventions on this document. Xxxxxx The start date appears in parentheses at the beginning of the description. If the start date is within the previous 9 months, the year is omitted.

New Page for each Care Plan

Select the check box to print each care plan on a separate page.

Print Entire Care Plan

Select the check box to print the entire care plan.

Discipline List

Select the disciplines you want to include on the report.
