Use post-discharge follow-ups within a client chart

This article details how to handle post-discharge follow-ups in the chart for a client receiving services. Note that setup is required to make these events available for use. 

Prerequisite:

Identify collaterals involved in the post-discharge follow-ups

  1. Go to: Client > Client Information > Relationships > Collaterals.
  2. Whether a Collateral is an Outside Organization Collateral, or a Personal Collateral, select the Post Discharge Follow Up Contact? check box to identify the person as a Post Discharge Follow Up Contact.
  3. Select Save.

Complete post-discharge follow-up events

There are two options for completing post-discharge follow-up events:

  • completion after the event is scheduled - Use this option if your agency uses workflows or schedules these events manually. 
    OR
  • completion if the event is not previously scheduled - Use this option if your agency does not use workflows or schedule events manually, and each event must be entered at the time of contact.

If you are unsure which option your agency is using, ask your system administrator.

Completion after event is scheduled

If your agency uses workflows or the event has been scheduled, it is best to use the Homeview to complete the event. This reduces the clicks and navigation, allowing for quick entry.

  1. To open the Homeview, click the myEvolv icon in the toolbar.
  2. Go to the My Tasks widget that lists all events and tasks to which the user is assigned.
  3. Select the name of the event.
    Result: The Post Discharge Follow up form displays.
  4. Complete the form and save.
  5. To modify or view this event after completion, go to: Client > Client Information > Critical Information > Post Discharge Follow Ups.

Note: It is possible for agencies to change the location where these events are filed.

Completion if event not previously scheduled

If your agency does not use workflows or schedule tasks to be completed, then do the following to complete the event:

  1. Go to: Client > Client Information > Critical Information > Post Discharge Follow Ups.
  2. Select New Manual Event, and choose the relevant Event.
    Result: The related form will open.
  3. Complete the form and save.

Monitor changes over time

One of the primary benefits of using post-discharge follow-up events is the ability to monitor changes over time. For example, during completion of a third post-discharge follow-up event the form includes a subreport that shows answers provided during the first two follow-up events. This provides a quick and easy way to see progress over time.
