Upload collected materials

Use the All Materials Collected form in myEvolv upload all materials associated with a client, including videos, documents, images, and more. 

Menu paths
  • Client > Client Information > Personal Information > All Materials Collected
  • Referral > Agency Referral > Testing/Screening > All Materials Collected
  • Program Referrals > Program Referrals > Testing/Screening > All Materials Collected
  1. Go to one of the above listed modules, then search for and select a client or referral.
  2. In the toolbar, click New Event and select any of the items listed. Some examples include consent for treatment, court authorization, previous school reports, and more.
  3. In the form, complete the fields as appropriate. 
  4. Depending on the event you're creating, you may need to upload or scan a document:
    • To attach a document, in the Attached Document field, right-click and select Upload File. Browse to and select the appropriate file.
    • To attach a scanned document, in the Document Scanned field, right-click and select Upload File. Browse to and select the appropriate file.
  5. Select Save.
